SUBCONTRACTS
REQUIREMENTS
If
subcontractors or consultants will be used in completing
the scope of work, the following information must be
furnished to prepare the agreement:
Subcontract
- Organization
name, address and type (corporation, foundation, etc.)
- Name,
address, telephone, fax and e-mail of the business
official
- Subcontractor's
Principal Investigator
- Names
of any personnel considered to be of primary importance
in the completion of the
work:
- A
clear and detailed statement of the work to be performed,
including
a. Personnel requirements
b. Time
schedule
c. Specific
technical guidelines
d. Descriptions
and due dates for reports and/or deliverables
- Sole
source justification, or evidence of competitive bids
- Itemized
budget with back-up
- Beginning
and ending dates of the work
- Current
indirect cost negotiation agreement (if indirect cost
is budgeted)
- Current
travel policy (if travel is budgeted)
- Subcontractor
identification of
any known officer who is an employee of The
Texas A&M University System (or subcontractor
statement that no known officer is an employee of
TAMUS)
- Subcontractor
IRS taxpayer identification number (Not required for
non-U.S. organizations)
- Compliance
forms (animals, humans, DNA, hazardous materials,
etc.), if required.
Consultant
- Name,
address, telephone, fax and e-mail
- Social
security number
- A
clear and detailed statement of the work to be performed
including:
a. Time schedule
b. Specific
technical guidelines
c. Descriptions
and due dates for reports and/or deliverables
- Sole
source justification or evidence of competitive bids
- Completed
Determination of Independent Contractor Status
form
- Itemized
budget of all costs for which the consultant may be
reimbursed
- Justification
for the rate charged by the consultant for professional
services
- Beginning
and ending dates of the agreement
- Vita
and/or qualifications of consultant
